— confirmation that he/she is not subject to a bankruptcy procedure opened by a competent authority;
— confirmation that he/she is not subject to a liquidation procedure opened by a competent authority;
— confirmation of paid taxes, contributions and other public dues issued by a competent authority in the country where the economic operator is registered;
— confirmation from the Registry of Offences for Committed Crimes by Legal Persons that a secondary sentence for prohibition of participation on public bidding procedures or awarding of public procurement or public private partnerships contracts has not been issued against him/her;
— confirmation from the Registry of Offences for Committed Crimes by Legal Persons, that a secondary sentence for temporary or permanent prohibition to perform a specific business activity has not been issued against him/her;
— confirmation that there is no legally effective judgment imposing a misdemeanour sanction involving prohibition to perform a profession, activity or duty, or prohibition to perform a specific activity, imposed against him/her.

Information and formalities necessary for evaluating if the requirements are met:
1. Proof of employment (M1/M2 forms or as appropriate) or copy of a cooperation agreement for 1 (one) person or statement for engaged technical staff or technical authority regardless of that whether they belong to the Economic operator, and Statement of the Economic Operator that the nominated person is trained for installation of equipment for detection and measurement of ionizing radiation for Part 4, Part 6 and Part 7.
2. Certified reference list from bidder or copy of contracts for realized minimum two deliveries of this or similar kind of the equipment in the past three years, with prices, dates, buyers (Contracting Authority or Economic operator), with providing a confirmation for executed deliveries issued from the recipients or if that kind of confirmation can not be provided from reasons beyond the control of the economic operator, only statement for realized deliveries from the economic operator is enough for for Part 1, Part 2, Part 3, Part 4, Part 5, Part 6, Part 7, Part 8, Part 9, Part 10, Part 11, Part 12, Part 13, Part 14 and Part 16.
Minimum level(s) of standards possibly required:
1. To have employed or hired technical staff, at least 1 (one) person for installation of equipment for detection and measurement of ionizing radiation for Part 4, Part 6 and Part 7.
2. To have at least 2 realized deliveries from this or similar type of equipment in the past three years, with prices, dates, buyers (Contracting Authority or Economic operator) for Part 1, Part 2, Part 3, Part 4, Part 5, Part 6, Part 7, Part 8, Part 9, Part 10, Part 11, Part 12, Part 13, Part 14 and Part 16.
