— Unternehmensdarstellung (mit dem Angebot mittels Eigenerklärung vorzulegen): mit Angabe der auf dem GSI Formblatt abgefragten Daten, wie z.B. Anzahl der Mitarbeiter in den letzten drei Jahren, etc,

— Eigenerklärung zur Eignung nach §§ 123, 124 GWB (mit dem Angebot mittels Eigenerklärung vorzulegen): Eigenerklärung, dass die Kenntnis nach §§ 123, 124 GWB unrichtig ist und die dort genannten Fälle nicht vorliegen — GSI Formblatt.

Einzureichende Unterlagen:

— Angaben zu Gesamtumsatz und auftragsbezogenem Umsatz (mit dem Angebot mittels Eigenerklärung vorzulegen): Erklärung über den Gesamtumsatz des Unternehmens sowie den Umsatz bezüglich der besonderen Leistungsart, die Gegenstand der Vergabe ist, jeweils bezogen auf die letzten 3 Geschäftsjahre - auf dem GSI Formblatt Unternehmensdarstellung,

— Nachweis einer Betriebs-/Berufshaftpflichtversicherung (mit dem Angebot mittels Eigenerklärung vorzulegen): Angabe des Versicherungsunternehmens und der Deckungssumme auf dem GSI Formblatt.

Zahlungsbedingungen: Abrechnung und Zahlung erfolgen in EURO. Die Zahlungen erfolgen erst, wenn die Zahlungsvoraussetzungen erfüllt sind und die jeweilige Rechnung vorliegt. Vorauszahlungen können nur gegen Vorlage einer unbefristeten, für den Auftraggeber kostenlosen Bankbürgschaft, nach Rechnungsstellung erfolgen.
