2) Erklärung zum Gesamtumsatz und zum Umsatz bezogen auf die Leistungsart, die Gegenstand der Vergabe ist, für die letzten 3 abgeschlossenen Geschäftsjahre (§ 122 Abs. 2 Nr. 2 GWB i.V.m. § 45 Abs. 4 Nr. 4 VgV);
3) Erklärung zur Berufs- oder Betriebshaftpflichtversicherung (§ 122 Abs. 2 Nr. 2 GWB i.V. m. § 45 Abs. 1 Nr. 3VgV).
Nachweis einer Betriebshaftpflichtversicherung mit einer Deckungssumme für Sach- und Personenschäden in Höhe von 1,0 Mio. EUR p.a. (zweifach maximiert), 500 000,00 EUR für Vermögensschäden pro Schadensfall durch eine Bescheinigung des Versicherungsgebers oder Bescheinigung eines Versicherungsgebers, dass eine entsprechende Versicherung im Auftragsfall abgeschlossen wird (jeweils Kopie ausreichend);
4) Auf gesonderte Anforderung der Vergabestelle: Jahresabschlüsse oder Auszüge von Jahresabschlüssen, falls deren Veröffentlichung in dem Land, in dem der Bewerber oder Bieter niedergelassen ist, gesetzlich vorgeschrieben ist.

— Angabe und Größe der Mitarbeiterentwicklung der letzten 3 Jahre (mit dem Angebot mittels Eigenerklärung vorzulegen);
— Referenzen (mit dem Angebot mittels Eigenerklärung vorzulegen): Referenzliste der letzten drei Jahre (Kunden in Deutschland mit Namen eines Ansprechpartners und Kontaktdaten).

5) Die angebotenen Geräte müssen den Forderungen des LaGetSi, den Vorschriften der zuständigen Berufsgenossenschaften (BGV A3 Elektrische Anlagen und Betreibsmittel) und den Anforderungen der Maschinenrichtlinie und des Gerätesicherheitsgesetzes für technische Arbeitsmittel in der zum Zeitpunkt der Lieferung gültigen Fassung entsprechen. Alle gesetzlichen Auflagen müssen erfüllt sein, einschlägige DIN- und VDE-Vorschriften sowie ggf. die Richtlinien für den Strahlenschutz sind zu berücksichtigen.
Es ist die neueste Gerätegeneration anzubieten.
