Description of the procurement:
AZT Turnhout wenst de lineaire versnellers momenteel in gebruik te vervangen door 2 high end lineaire versnellers die volledig uitwisselbaar zijn. De versnellers moeten uitgerust zijn voor IMRT, IGRT, DGRT, ART, GatedRT, SBRT en frameless SRT. Het huidige verificatiesysteem en de huidige planningssystemen dienen te worden uitgebreid of te worden vervangen. De veiligheid en kwaliteit van de bestraling dient maximaal te worden gegarandeerd door randapparatuur voor kwaliteitscontrole van de versnellers en monitoring van de patiënt en de behandeling zelf. Verder wordt er gestreefd naar maximale automatisering en een optimale workflow.

Het UEA, waarmee de inschrijver verklaart dat hij voldoet aan de onderstaande selectiecriteria:

1. Gecontroleerd en goedgekeurd door een Belgische erkende instelling voor fysische controle. De inschrijver dient te voldoen aan alle vigerende wetgeving en normering en te beschikken over de nodige erkenningen voor de domeinen van toepassing.

2. Aanleveren kopie (in het Nederlands) van EG Verklaring van Overeenstemming met volgende gegevens:

— firmanaam en volledig adres van de fabrikant en, in voorkomend geval, diens gemachtigde

— productidentificatie (naam, de functie, het model, het type, het serienummer en de handelsbenaming en alle relevante aanvullende informatie)

— de richtlijnen die zijn nageleefd en in voorkomende geval de geharmoniseerde normen of andere gehanteerde technische normen en specificaties

— in voorkomend geval naam en adres van degene die gemachtigd is het technisch dossier samen te stellen; deze persoon moet in de Gemeenschap gevestigd zijn

— in voorkomend geval naam, adres en identificatienummer van de aangemelde instantie

— identiteit en handtekening van degene die gemachtigd is om namens de fabrikant of diens gemachtigde de verklaring op te stellen

— de datum waarop de verklaring is afgegeven
